To sign up for our classes as a MODEL, please do the following:

1. Fill out the linked form below with your information, and make sure to include your photos using the upload button on the form
2. BEFORE YOU PAY, RECEIVE APPROVAL BY ONE OF OUR TRAINERS – THEY WILL CONTACT YOU DIRECTLY USING THE CONTACT INFORMATION YOU PROVIDED ON THE MODEL APPLICATION FORM
3. UPON APPROVAL, submit payment via ONE OF THE FOLLOWING OPTIONS:
   • Zelle to [email protected], and INCLUDE YOUR FULL NAME in the Memo field of your payment, OR
   • Purchase LIP FILLER MODEL ITEM HERE if you are modeling for LIPS OR
   • Purchase FACIAL BALANCING MODEL ITEM HERE if you are modeling for facial filler.
   IF YOU HAVE NOT BEEN APPROVED BY ONE OF OUR TRAINERS, DO NOT PROCEED WITH THE PAYMENT
